Community Support Services

Getting Assistance

Do you need assistance with any of the following (in or out of your home)?

  • Housing
  • Getting linked to needed services in the community
  • Help to learn and use daily living skills (grocery shopping, transportation, managing finances, learning/using coping skills, etc.)

Community Support Services provide assistance to meet these needs.

Community Support Services

Community Support Services provides education, linkage, and case management to you, your family, and other important supports in your life so that you can learn to manage your symptoms, stay in the community and out of the hospital, and live as successfully and independently as possible. These services come to you, in your home and community. Services include skill-building tools for daily living, education about mental illness and medication, development of community supports, and case management/service coordination.

Community Support Services are available to children, adolescents, and adults with mental illness and/or addictive disease in all 10 of the counties we serve.
